Overview
Genderbending is a phenomenon wherein a character changes gender by any means. The past tense and adjectival forms are both "genderbent". Sometimes a person who frequently changes genders is called a "genderbender". The term "transsexual" is, for whatever reason, rarely used.
MTF
This stands for male-to-female and is by far the most common type of change in role-plays.
FTM
This stands for female-to-male and is rare in role-plays.
Surgery
This never happens in a medieval setting, though is possible with time-travelling (to/from the future) or planehopping (to/from a technologically-advanced world). Going by Earth's scientific abilities only, a surgical transsexual should be unable to have offspring as the new gender (FTM cannot insemenate; MTF cannot ovulate).
Magic
This is by far the most prevalent cause of switched genders. It can be caused by a magical piece of jewelry or clothing that only functions when worn, a potion, a spell, a curse, a wish, etc. Usually the effect is permanent until either 1. magical garment is removed, or 2. a potent magic-user reversed the effect, but sometimes it wears off on its own.
Mostly, a magical genderbend will only change the character's sex superficially and not all the way down to the genetic level, as would be the case with surgery. Some particularly potent spells will change it on the genetic level, and some even retroactively alter the past so that the affected character has always been the other gender.
Regardless of exact effect or how "deep" the change goes, a genderbent person of this type will almost invariably be instantly fertile as the new gender and can begin conceiving offspring within minutes.
Hermaphrodites
Some extremely rare characters exist as both genders at once. They were either born that way or made that way somehow after birth.
